Faculty & Staff Driver Approval Certification
Welcome to the Princeton University Faculty and Staff Driver Approval Certification page. Follow the steps on this page to register for and complete the Driver Safety Certification process. If you have any questions or issues, please select Support at the top of this page and open a ticket.
Select each of the buttons below to download the Driver History Questionnaire and the Vehicle Usage Agreement. After you have downloaded these forms, print the forms, complete the forms, and sign them.
When you have finished filling out and signing the forms, scan each form or take a clear photo of each form. Enter your first name, last name and email in the fields below. Select your identifying group and your department. Then, upload your signed Driver History Questionnaire and your signed Vehicle Usage agreement.
Click Submit to begin the certification process. The forms will be forwarded to Risk Management so they may complete a Motor Vehicle Records check, which may take up to 5 business days. You will receive email notification when your driving privileges have been approved or if you need to provide additional information.
